Operation principle

The ultrasonic scalpel is composed of main components, such as a host, a handle, a knife head, and a foot pedal. There is a transducer in the handle, which converts the high-frequency electric energy provided by the main engine generator into ultrasonic mechanical vibration energy, and transmits it to the cutter head while transmitting the ultrasonic energy to hemostatically cut or coagulate the tissue. The vibration frequency of the cutter head It is about 55.5kHz.

The high-power ultrasound can make the tissue cells in contact with the cutter head instantly vaporize water, break the protein hydrogen bond, and disintegrate the cells to cut the tissue. The frictional heat caused by mechanical vibration can coagulate and stop bleeding while cutting the tissue. Correct Different

The ultrasonic cutter head has two leaves, one is the working blade for applying ultrasonic waves, and the other is provided with a white protective pad with tooth patterns. The function of this white pad is to avoid the direct contact of the ultrasonic blade with the metal, which will cause noise and damage to the blade. At the same time, it can also help the surgeon better hold the tissue and enhance the coagulation effect. The working blade can be used to cut directly like an electric knife .

Clinical application

1. Classification of ultrasonic knife heads There are many types of ultrasonic knife heads, which can be selected according to the usage. According to the size, it is divided into 5mm and 10mm cutter heads. Among them, the 10mm cutter head has three kinds of cutter faces: sharp, blunt and flat; according to the shape, it is divided into curved cutter head and straight cutter head.

2. The advantage of the ultrasonic knife is: Compared with the unipolar electrosurgical knife, it uses a high-frequency current generator to generate high-frequency current to generate high temperature locally in the body tissue, so as to achieve the purpose of coagulation and hemostasis.

Ultrasonic knife is part of mechanical damage and thermal damage. Compared with electric knife, ultrasonic knife has obvious advantages:

(1) The function of separating and cutting hemostasis can be completed at one time, reducing equipment replacement.

(2) The working temperature of the ultrasonic knife is much lower than that of the electrosurgical knife and the laser (high frequency electrosurgical knife 150℃, laser 350°C), which produces less heat and will not damage the surrounding tissues due to thermal damage. The operating temperature of the commonly used electric knife during electrocoagulation is 150~400℃, the thermal effect is 2.5~4mm, and the surrounding tissues are easily damaged. When the ultrasonic knife is used, the local temperature is lower than 85 ℃, and the thermal damage is less than that of the traditional electric knife.

(3) When cutting, the smoke generated by the ultrasonic knife is relatively small, and the visual field is less affected during laparoscopic surgery.

(4) This instrument can be used as an auxiliary or substitute for electrosurgery, laser and scalpel. It is suitable for general surgery, plastic surgery, pediatrics, obstetrics and gynecology, urology, thoracic surgery, and exposed bony structures (such as spine and joints) surgery and other open and endoscopic surgery.

(5) Since the ultrasonic knife adopts the principle of ultrasonic cutting and coagulation, there is no current passing through the body, and no conductive tissue damage will occur. When working, only the knife head contacts the patient's part.
