laparoscopic endoscopic surgery journal(3)-Instruments

The operation of laparoscopic surgery is performed in a relatively closed "cavity", and many spaces are artificially formed, and there is no direct passage with the body surface, so its special equipment is designed.

(1) disposable laparoscopic trocar

During laparoscopic surgery, most of the cavities for observation and operation are not directly connected to the body surface. The establishment of the cavity space is mostly artificially formed by liquid or gas. In order to ensure the smooth operation, the trocar Bears the passage between the outside and the cavity. The trocar is composed of a puncture sleeve and a puncture core. The puncture core is inserted into the sleeve. After the skin is cut at the corresponding part of the body surface, the trocar is inserted from the body surface, and the inner core is pulled out to establish a channel . The puncture sleeve has two sets of ports communicating with the cavity, and the ports are equipped with valves.

The instrument channel allows the endoscope or instrument to enter the cavity from the outside conveniently. The other interface can be used to fill the cavity with liquid and gas. In addition, there is a valve to keep the cavity space relatively airtight, and there can be a relatively constant during the operation Space. Surgaid has 5/10/12 mm trocar for your choice.

(1) other instruments

Instruments for endoscopic surgery are generally between 2 and 12 mm in diameter. The operating handle is the same as traditional instruments (Figure 51-2-5). Depending on the role, the functional heads are diverse, and the appearance and instrument functions are the same as traditional instruments. The name also depends on the function of the head, such as grasping forceps, scissors and needle holder. The most distinctive feature is that there is a long transmission rod between the head and the handle. The functional head of the operating handle rotates, opens, closes or more complex requirements are sent to the functional head to achieve the purpose of operation, thereby solving the problem of endoscopic surgery. Surgical instruments can only reach the surgical area deep in the cavity through the trocar, and the manipulation of the instruments is outside the cavity. Because the instruments must be in and out of the trocar, all instruments are cylindrical except for the handle. Except for the functional head, most of the instruments are insulated from the whole body. There is an electrode interface at the operating handle, and an electric knife is connected so that the head can have the function of electrocoagulation or electrocutting during operation. In order to facilitate the operation under the endoscope, some instruments have made necessary improvements to adapt to the use under the endoscope, such as electrocoagulation hooks. When laparoscopic surgery is used to tie knots or sutures to close pipes and tissues, titanium alloy clips or polymer composite clips are generally used instead, which is convenient for operation and saves time.
